Mediation Checklist

If you a considering Mediation as a possible solution, here is a helpful checklist of documents and  information you should bring.

Documentation necessary for a successful family mediation are:

Real Property

An agreed upon value of any real estate involved or, if that is not possible, a recent appraisal of the property. We can recommend local appraisers upon request.  This documentation is crucial for a successful mediation.

Recent statements for:

401(k)s

If one or both of the participants had funds in a 401(k) or pension plan prior to the marriage, a statement date closest to the date of marriage will be helpful.

Pension plans

If either party has a Pension Plan a recent statement will be helpful

Credit Report

A list of creditors, to include current balances. A current credit report can help make accurate lists.

Affidavit of Financial Means

If minor children are involved, both participants are required to complete affidavits of financial means prior to the mediation.

Mediation Questionnaire

Review the Mediation Questionnaire to identify additional assets you may have forgotten.

Alimony

If one of the participants is in need of spousal support, the participants will both need to exchange the affidavits of financial means to assist in the discussion of the issue.
